Dr. Cook earned his Doctor of Medicine Dentistry (DMD) from the University of Louisville School of Dentistry in 1995

Accomplishments

Served six years as an officer in the US Navy as a general dentist, where he attained the rank of Lieutenant Commander.
In 2003, Dr. Cook opened his first practice in southern Indiana. He has since bought and sold multiple practices.
Received specialty training at the University of Louisville Graduate Endodontic Program.
Nationally recognized lecturer that focuses on continuing education courses in General Dentistry and Endodontics
Teaches undergraduate and postgraduate students at the University of Louisville School of Dentistry on Endodontics
Member of the American Association of Endodontics, the American Dental Association, the Kentucky Dental Association, the Louisville Dental Association and the past president of the Southern Indiana Dental Association
